Massage Health Intake form: We collect personal information about your current and past health as well as your massage experience and preferences. This information is only shared with the Licensed Massage Practitioner who will be serving you in order to ensure your safety and so she can give you the most customized massage experience possible.
- On this form, we also collect your name, phone number, mailing address, birthday and email address. This information is shared with our secure email marketing platform sendinblue (www.sendinblue.com). We may use your email address to send you updates, special offers at the holidays or during your birthday month. We may send you 1 or 2 physical cards per year at the mailing address you provide. If at any time you wish to not receive emails from Self-Transformations.com or ReflectiveCoaching.net, you can opt out at the bottom of any email you receive from us. If you wish to no longer receive physical cards, please email us at firstname.lastname@example.org and we will take you off our physical mailing list. We DO NOT sell or share your information with any third party.
Our Online scheduling platform: In order to schedule online, we collect the bare minimum of personal information, your name, phone number and email address. These are necessary to send out automatic reminder emails and the occasional SMS message prior to your appointment. You may opt out of these emails by emailing us at email@example.com.
Online Checkout and Dashboard: Users may choose to pay for massage services online and create an account in order to track packages, pay for massage services and schedule upcoming appointments as well. This information is never sold or shared with third parties and is secured using the online secure payment platform Stripe (www.stripe.com). Activity: We may record information relating to your use of the Site, such as the searches you undertake, the pages you view, your browser type, IP address, requested URL, referring URL, and timestamp information. We use this type of information to administer the Site and provide the highest possible level of service to you. We also use this information in the aggregate to perform statistical analyses of user behavior and characteristics in order to measure interest in and use of the various areas of the Site.
